Clinical Evaluation of the Spinal Cord Tumor



Abstract
A total of 55 patients of spinal cord tumor who had been treated by operation at the Department of Neurosurgery of Chonnam University Hospital between January 1981 and May 1987 was analyzed clinically.
The results were summarized as follows:
1) The period of highest incidence was between 40 and 60 years of age(56.4%) and the ratio of male to female was 1.3:1.
2) The most common spinal cord tumor was neurinomas comprising 43.6 percent of all spinal cord tumors, with metastatic tumors(21.8%) next followed by meningiomas (16.6%). gliomas(5.5%), and granulomas(5.5%).
3) The most common site was the thoracic region(65.5%), with the lumbosacral region next(21.8%) and the cervical region(12.7%) the least likely site for tumors.
The location of the tumors were intradural-extramedullary(52.7%), extradural(40.0 %), and intramedullary(7.3%).
4) Motor weakness(70.9%) was the most common presenting complaints at the time of admission. and less common clinical features were sensory disturbance(63.6%), pain (58.2%), and sphincter disturbance(43.6%).
5) There were abnormal findings in 50.9% of all spine films. The whole 53 cases of myelography showed either complete or partial subarachnoid block. The computerized tomographic metrizamide myelography(CTMM) delineated in detail the relationship of the tumor with the spinal cord and the dura.
6) Total excision was performed in 29 cases(52.7%) of all spinal cord tumors: all cases in 9 meningiomas, 20 cases(83.3%) in 24 neurinomas, and a glioma out of three etc.
7) The results at time of discharge showed that 7 cases(12.7%) were considered cured, 26 cases(47.3%) improved, and- 22 cases(40. 0%) did not benefit from the operation. It was excellent particularly in case of intradural-extramedullary tumors.
8) Radiotherapy was thought to have considerable effect in relieving spinal pain of a metastatic lesion.
